Bradley Stolz-Grobusch (ABN: 19037872308, Sole Trader, Est. 2009) collects personal information about you in order to provide you with a health service. This privacy policy is to provide information to Bradley Stolz-Grobusch’s clients as to how their personal information (including health information) is collected and used, and the circumstances in which it may be shared with third parties.

When you register as a client of Bradley Stolz-Grobusch, you provide consent for Bradley Stolz-Grobusch and their employed or contracted practitioners and staff to access and use your personal information so that they can provide you with the best possible healthcare. Only employees or contractors who need to see your personal information will have access to it. If we need to use your information for anything else, we will seek additional consent from you to do this.

Bradley Stolz-Grobusch will need to collect your personal information to provide healthcare services to you.   • Medicare number (where available) for identification and claiming purposes
  • Healthcare identifiers
  • Health fund details, if applicable
  • Credit card details

Bradley Stolz-Grobusch will collect your personal information:

  • When you make contact for your first appointment, your personal and demographic information will be collected via your registration
  • When you sign a consent form agreeing to treatment, your personal and demographic information, including that of your next of kin and/or emergency contact, will be collected via your signed agreement
  • During the course of providing healthcare services, we may collect further personal information
  • We may also collect your personal information when you send us an email or SMS, telephone us, make an online appointment, or communicate with us using social media

In some circumstances, personal information may also be collected from other sources. Often this is because it is not practical or reasonable to collect it from you directly. This may include information from:

  • Other involved healthcare providers, such as specialists, allied health professionals, hospitals,

  • For the purpose of confidential dispute resolution processes
  • When there is a statutory requirement to share certain personal information (such as some diseases or behaviours which may require mandatory notification)

Only people that need to access your information will be able to do so. Other than in the course of providing healthcare services or as otherwise described in this policy, Bradley Stolz-Grobusch will not share personal information with any third party without your consent. Bradley Stolz-Grobusch will not use your personal information for marketing any of our goods or services directly to you without your express consent. If you do consent, you may opt-out of direct marketing at any time by notifying Bradley Stolz-Grobusch directly.

Bradley Stolz-Grobusch stores all personal information securely and in accordance with the policies of the practice management software companies utilised. Bradley Stolz-Grobusch and these companies comply with the Australian Commonwealth Privacy Act 1998 together with the Victorian Health Records Act 2001 and their applicable principles when handling personal or health information. You have the right to request access to, and correction of, your personal information. Bradley Stolz-Grobusch acknowledges clients may request access to their health records. We require you to put this request in writing and provide it in person, by mail or by email. All requests must be accompanied by photo ID for verification purposes. Our practice will respond within 28 working days. There is an administration cost of $39.70 for a full copy of medical records and up to $149.50 for a series of test results or health summary.

Bradley Stolz-Grobusch will take reasonable steps to correct your personal information where the information is not accurate or up to date. From time-to-time, we will ask you to verify your personal information held by our practice is correct and up to date. You may also request that we correct or update your information, and you should make such requests in writing.
